How to upload a prescription for EpiMonitor

As EpiMonitor is an FDA-cleared medical device, if you purchase it within the US territories, you will be required to submit a US prescription directly to your EpiMonitor Account.

 

We have created a prescription template with the required info that you can share with your doctor, to download it, click here. If you would like to learn more about the prescription requirements, please visit Prescription Requirements for EpiMonitor.

 

After you have placed the order, you will receive the account creation link in your email inbox to set up your EpiMonitor Account. You can find the step-by-step guide to creating the EpiMonitor Account here: How to create your EpiMonitor account.

 

Once you have completed your account creation, you can then upload your prescription by following these steps:

  1. Log in to your EpiMonitor Account.
  2. On the main page of the account, you will see the message ‘Order frozen’ due to the missing prescription. Click on the arrow next to the    icon and then press the 'Add prescription’ button.

  3. A new window will pop up and you can now upload your prescription by clicking on the ‘Add the prescription’ link.

  4. Select your prescription file directly from your computer/smartphone or take a picture with your smartphone (we accept only PDF, PNG, and JPEG file formats, and the file size must be less than 10MB).
    Alternatively, you can also drag and drop the file to the selected area.

  5. Review the uploaded prescription and click ‘Next’.

  6. Fill in the form with the prescription information. You should provide:
    1. Prescription issue date: the date when your prescription was issued

    2. Physician name: the full name of the doctor who prescribed EpiMonitor

    3. National Provider Identifier (NPI): this is a unique ID assigned to healthcare providers. You can find it on your prescription

    4. Physician address: the full address of your doctor's office as written on your prescription

  7. Please make sure that the information you have entered matches the information on the prescription.

  8. Click on Upload prescription in the bottom right corner.
    Please make sure the prescription has all the requested information and that the information you enter matches what is on the prescription.

  9. If your prescription has been submitted successfully, you should see the message ‘Prescription under review’ when you return to the main page.

  10. If your prescription is approved: You will receive a confirmation email within 24 hours. The prescription status will be automatically updated inside the EpiMonitor account, and your order will be placed automatically in the next shipping batch, ready to be shipped out.

    If your prescription is rejected: You will receive an email with the rejection reasons. The prescription status will be updated inside the EpiMonitor account. You can then make the necessary changes and resubmit it through your EpiMonitor Account.
